About the Role:

Under the direction of the Director of Youth and Family Services, this position is responsible for managing the care of individuals on the caseload in the TIP (Transition to Independence Process) Program serving adolescents and young adults ages 14-29 with emotional/behavioral difficulties using a model similar to an FEP or ACT model in a creative, wrap around service. Transition Facilitator Team Lead will teaching skills to prepare individuals to live independently, obtain employment, advance education or functioning in the community more effectively while conducting home-based, school-based, or field-based service delivery.

Transition Facilitator Team Lead provides on-site and off-site support to individuals in community-based employment or in school. The Team Lead will also run weekly team meetings, assign cases, conduct OP therapy, MHSS assessments and make DARS referrals. Based in Oakton, VA. Services may be provided to youth and young adults in the NOVA area.

  • $70,000- 75,000 Estimated starting salary based on qualifications and related experience

Responsibilities:

  • Lead weekly team meetings and oversee case management for clients in the TIP (Transition to Independence Process) Program.
  • Provide individual, group, and family therapy, along with psychoeducational services.
  • Conduct assessments for TIP, MHSS, and OP therapy, developing individualized rehabilitation/transition plans with clients.
  • Facilitate discharge planning and coordinate after-care services for clients leaving hospitals.
  • Act as a liaison with other care providers to ensure comprehensive and coordinated care.
  • Deliver evidence-based psychotherapy to support clients in transitioning to community-based activities.
  • Conduct risk assessments, provide crisis intervention, and offer dual diagnosis-focused services.
  • Maintain accurate and detailed client records according to licensing and Medicaid standards.
  • Demonstrate flexibility to work outside of business hours to meet client needs.
  • Continuously develop your professional skills through training opportunities.

Qualifications:

  • Master’s degree in psychology, social work, rehabilitation counseling, or a related field with significant experience.
  • Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW), or approved by DHP for licensure supervision.
  • Bilingual English and Spanish preferred.
  • Strong understanding of treatment approaches for emotionally and socially disturbed individuals, with knowledge of relevant psychiatric, psychological, and sociological concepts.
  • Excellent written and presentation skills, with the ability to build rapport with clients, internal staff, and external providers.
  • Ability to work independently with strong organizational skills.
  • Experience with electronic health records (EHR) or a willingness to learn quickly.
  • High degree of empathy and a passion for helping young adults overcome challenges.
