We are looking for a Social Worker for the Social Services Department at St. Mary Medical Center.
Location: Apple Valley, CA
Work Schedule: Full time – 80 Biweekly Hours
Shift: 8-hour, Days
Job Summary:
Provides psychosocial assessment, crisis intervention, referrals and information to patients and families. Provides education/support services to patients, families and staff. Works collaboratively with the interdisciplinary team to effect positive patient and family outcomes and to ensure that resources are in place to facilitate the discharge plan. Serves as resource for less experienced social workers.
Essential Functions:
- Supports the values and mission of St. Mary Medical Center and practices the core values when interacting with those served by the Medical Center.
- Assesses social, emotional and financial needs of patients/families; identifies patient/family resources required to facilitate the discharge plan.
- Identifies and participates in problem solving issues that could be discharge barriers from the psychosocial perspective.
- Provides crisis intervention, and initiates referrals to appropriate resources to ensure patient/family psychosocial and emotional needs are addressed.
- Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare continuum and assists in identifying and facilitation system improvements.
- Works with leadership to identify problems, recommends solutions and works toward resolution.
- Identifies need for and conducts family meetings and/or interdisciplinary team conferences to resolve issues and ensure treatment/discharge plan progresses.
- Manages issues such as guardianship, abuse/neglect, adoption, power of attorney, healthcare advocacy, surrogacy, and advanced directives.
- Serves as point person for issues regarding end of life, ethics, and pain management.
- Serves as resource to hospital staff and physicians regarding emotional, social and psychosocial components of patient illness. Ensures integration of the interdisciplinary team in understanding and integrating these aspects into the plan of care.
- Documents activities with entries in the patient’s clinical record that is consistent with meeting department and hospital requirements, licensure, and regulatory guidelines.
- Supports interdisciplinary team conferences and contribute to the planning and review of care for patients presented as needed.
- Follows organizational and departmental procedures.
- Maintains collaborative relationship with interdisciplinary healthcare team and community service.
- Adheres to standards, policies and procedures at both the hospital and department level.
- Promotes a service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff and visitors.
- Functions in an environment that recognizes the spiritual component of life.
- Contributes to the cohesive functioning among social workers and chaplains.
- Participates in weekend rotation and on call as required.

- Complies with Title 22 and other state, federal and TJC regulations as they pertain to this position and department.

Minimum Position Requirements:
Education:
- Master’s Degree in Social Work from accredited university.
- Coursework/Training – Courses or continuing education in Evidence-Based Intervention.
Experience: 1 year Social work experience in a health care setting (outpatient/acute care or skilled nursing facility).
Licenses/Certifications:
- Registered with CA Board of Behavioral Sciences as an ASW due within 90 days of hire.
- LCSW Designation preferred by evidence of Primary Source Verification.
- LCSW licensure with BBS due within 4 years of hire.
- BLS Certification.
