Position Overview

Under general supervision of the Unit Administrator and clinical direction of the Chief of Social Work, performs extensive casework in the role of Treatment Coordinator for patients in the rehabilitation program. Conducts individual and group therapy on behalf of the patients. Reviews social histories of admission; verifies social assessments are completed. Interviews assigned patients relatives and appropriate collateral resources Serves as a member of a multi-disciplinary treatment team, develops, designs, implements and evaluates treatment plans. Serves as a member of various Performance Improvement Teams.

Job Responsibilities

  • Performs extensive casework in the role of Treatment Coordinator for patients in the rehabilitation program. Attends treatment team meetings.
  • Conducts individual and group therapy sessions on behalf of the patients, utilizing professional social work skills with the intent of successfully facilitating transfer to a less secure environment. Assesses responses to sessions and documents therapeutic responses according to standards and facility policy.
  • Reviews social histories of admission to verify information is complete and accurate.
  • Interviews assigned patients relatives and appropriate collateral resources to formulate and document a complex, comprehensive social assessment of patient.
  • Serves as a member of a multi-disciplinary treatment team, develops, designs, implements and evaluates treatment plans. Serves as a member of various Performance Improvement Teams and committees commensurate to the Social Workers function at Chester Mental Health Center.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

 


Minimum Qualifications

  • Requires a master’s degree in social work from a recognized college or university supplemented by one year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled or other disabled persons.
  • This class is included as an Upward Mobility Program credential title.

Conditions of Employment

  • Requires the ability to meet all agency vaccine/health-related policies and guidance.
  • Requires the ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
