Essential Functions
- Provide comprehensive Elderly Waiver or Disability Waiver case management services to clients under one of our contracts: Blue Plus, Medica, UCare MSC+ and MSHO contracts, or Disability Waiver Contract with Ramsey and Hennepin County
- Provide care coordination services including assessment and service planning, resource referral and collaboration with other providers involved in client care
- Provide skill building techniques and a supportive relationship to foster trusting relationships
- Participate in team and program staff meetings and contract required meetings
- Complete documentation in electronic record systems as required by program and contract
Qualifications
- Bachelor of Art or Science in Social Work.
- MN Board of Social Work Approved License (e.g. LSW, LGSW).
- Prefer experience in case management and/or knowledge of waiver services.

Pay: $51,000.00 – $57,000.00 per year
