The Part-Time Social Worker will work 24 hours per week and is responsible for overall care coordination of patient and family, from admission through discharge. The Social Worker guides the family through continuum, acting as liaison between family, multidisciplinary team, physicians and external agencies. He/She monitors family and patient’s understanding and compliance with treatment and discharge goals and coordinates team functions toward achievement of discharge plan. The Social Worker maintains contact with third party to assure ongoing funding of care and maximum utilization of services available, to assure the child’s ongoing care.
Pediatric experience is not required, but preferred. Willing to train the right candidate.
