Social Worker
Full-Time – Day Shift (M-F, 8-4:30pm)

Job Summary
In collaboration with a multidisciplinary team, this role completes intake assessments, assists with treatment planning, monitors progress, facilitates patient and family meetings, and supports discharge services. This role provides psychotherapy utilizing brief therapy models incorporating crisis stabilization and cognitive behavioral techniques. Participates in health quality improvement and evaluation processes. Position supports discharge and aftercare services that support patient self-management, adherence to treatment plans, and reduce unnecessary utilization. Through an integrated approach, conducts home visits, office visits, and telephonic follow-up for patients and their families to navigate and access community-based services and resources.

What You Need to Be Successful:
- Maryland licensure of LMSW required.
- Two years of relevant clinical experience; three years is preferred.
- Experience working with individuals with acute, chronic, and social needs required.
- Acute care social work experience preferred.
- Knowledge of Care Management preferred.
- Understanding of insurance eligibility and coverage requirements.
