Job Overview
The Rehabilitation Specialist will supervise staff in accordance with company policies & procedures, as well as State & Federal laws. Responsibilities include, but are not limited to interviewing, hiring & training employees. The addressing of complaints & resolving problems. Maintaining quality improvement goals as well as addressing various concerns at scheduled staff meetings. Overseeing client delivery, insure performance measurements, & outcomes are met.
- Complete rehabilitation assessments to identify the rehabilitation needs of participating consumers.
- Lead & coordinate rehabilitation program team meetings with engaged stakeholders, including therapists & consumers.
- Develop individual Rehabilitation Plans for each consumer, with domains, short term & long term goals with intervention.
- Provide orientation, supervision, & training to PRP workers to assure the support & goal attainment of participating consumers.
- Shares/Transmits the mission & vision of Pinnacle Enrichment Healthcare Services, LLC to staff, clients, & families served.
- Maintain manageable caseload to insure outcomes & participation requirements are met while taking into consideration the level of care of clients served.
- Perform all duties within the contract, state & federal regulation standards.

Pay: $1,500.00 per month

Experience:
- working with adults with mental health conditions: 3 years (Preferred)
License/Certification:
- Social Work License (Preferred)
- CPR Certification (Preferred)
