Job Overview
* SUMMARY*:
The Qualified Mental Health Professional II (QMHPII) Navigate Team is a member of the multidisciplinary team and has the primary responsibility to provide case management and supported employment and education services to individuals involved with the Navigate team. Navigate is a comprehensive program designed to provide early and effective treatment to individuals who have experienced a first episode of psychosis. The QMHP II works with the individual to identify his or her personal preferences regarding work and school goals, including career interests, and then provides the necessary supports to help the person achieve those goals.
ESSENTIAL FUNCTIONS:
- Engage individuals in school and/or work interests, particularly competitive employment, educating them about opportunities and benefits of work or school; facilitate individualized job placement according to client preference; provide ongoing coaching and support to clients in vocational and/or educational pursuits.
- Conduct pre-vocational assessment focused on client strengths and preferences, as well as on-the-job assessments, where appropriate. Facilitate individualized job placement according to the individual’s preferences, with a focus on competitive employment; provide job coaching and ongoing supports, assisting the individual in learning the job skills, navigating the workplace, managing work relationships with other employees and supervisor.
- Conduct job development by building relationships with local businesses and educating them about the services that the vocational specialist provides, collect information about positions, and determine potential for job options. Serve as liaison with community partners to support functioning in work or education environment.
- Provide case management and advocacy services to individuals/families on the Navigate team.
- Document direct service and collateral contacts in the electronic health record. Meet timeliness of documentation expectations.
- Follow established Agency methods and procedures for documentation of progress notes, individualized treatment plans, and quarterly reviews in the electronic health care record.
- Attend weekly Navigate team meetings, formal supervision, and trainings.
OTHER DUTIES:
- Provide transportation to individuals as needed
- Perform such other duties as assigned by supervisor
QUALIFICATIONS:
- Knowledge of services and systems available in the community including primary health care, support services, eligibility criteria and intake processes, and generic community resources.
- Knowledge of mental health
- Skills in identifying and documenting an individual’s need for resources, services, and other supports.
- Skills in using information from assessments, evaluations, observation, and interviews to develop service plans.
- Ability to engage in and sustain ongoing relationships with individuals in services.
- Good oral and written communication skills.
Minimum Requirements
EXPERIENCE/EDUCATION REQUIRED:
Bachelor’s degree in a human services field with at least one year of associated work experience.
Must meet Board of Health Professions registration requirements for Qualified Mental Health Professional (QMHP-A) or Qualified Mental Health Professional (QMHP-A) Trainee.
