Job Overview
This is a full-time position in the Intellectual & Developmental Disabilities (IDD) Administrative Entity (AE) Unit of Lancaster County Behavioral Health & Developmental Disabilities. This employee will need to possess both programmatic skills as well as the ability to complete difficult and complex tasks as it relates to eligibility for IDD services and supports. Activities include reporting and monitoring information related to eligibility for IDD Support Coordination, Medicaid Waiver, and OBRA. This individual will not only be working with the IDD Supports Coordination Organization (SCO) Unit in Lancaster County, but also with the Office of Developmental Programs (ODP), the County Assistance Office (CAO), individuals, families and service providers as required and appropriate. The employee will use the Home and Community Service Information System (HCSIS) for programmatic activities. Work is performed with considerable independence under the supervision under the Director of AE.
REPORTING RELATIONSHIPS
Work is reviewed by the AE Admin Director through regularly scheduled individual and group conferences, and the review of records and reports.
ESSENTIAL JOB FUNCTIONS
1. Request and review needed documentation to complete certification, recertification, and waiver eligibility and enrollment activities for the Intellectual Disabilities Program.
2. Complete Intake and Referral activities in the absences of other staff able to complete these tasks. Assist with Intake and Referral acivities in accordance with demand.
3. Serve as OBRA coordinator and work collaboratively with appropriate agency personnel (i.e. Office of Aging, ODP, SCO) and complete appropriate documentation.
4. Operate and navigate HCSIS and CARES to gain access to needed reports and other databases to monitor Level of Care and completion of recertification.
5. Operate and navigate HCSIS and CARES to complete referrals and intakes under Intellectual Disabilities.
6. All assigned tasks and subsequent work must adhere to ODP and AE operating agreement requirements as well as internal BHDS and county policies.
7. Serve as the in/out of county transfer coordinator
OTHER SPECIFIC TASKS OR DUTIES
1. Will work with the AE and SCO staff to promote a cooperative work effort within the unit.
2. Provides own transportation to and from individuals’ home, meetings, outreach visits, and other job-related appointments in a timely manner and be able to access those locations with reasonable accommodations.
3. Complete other duties as specified by the Director of AE or Deputy Director of IDD.

MINIMUM QUALIFICATIONS
Two years of professional level experience in the field of intellectual and developmental disabilities; and a bachelor’s degree; or Any equivalent combination of experience and training..

Intellectual Disability – must have advanced knowledge of field of expertise including eligibility requirements and clinical definitions, as well as related fields, e.g. Medicaid Waiver, Mental Health, Person-Centered Planning, Self-Determination, etc. Must also have knowledge of services available.
REQUIRED LICENSES/CERTIFICATIONS/CLEARANCES

All clearances are required prior to the start of employment and may not be more than one year old at the time of employment.
- A Valid Driver’s license and acceptable driving history in accordance with County policy.
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
Program Planner/Evaluator must be able to work in a busy and open enviornment. Program Planner/Evaluator must be able to use a computer for data entry. This position has been identified as having the potential for exposure to Hepatitis B. Access to training and a voluntary vaccination against Hepatitis B is provided. Program Planner/Evaluator must be able to routinely provide his/her own transportation to and from consumer’s homes, meetings, outreach visits and other job-related appointments in a timely manner and be able to access those locations with reasonable accomodations.
