We are accepting applications for Peer Health Coach to provide outreach and support to clients on-site and off-site in the Fairfax Country community.

- Schedule: Monday – Friday, 8:30 am – 4:30 pm Location: Fairfax County, VA
In this full-time Peer Health Coach role, you will:
- Coordinate and complete health assessments within tight deadlines;
- Complete client notes/records as required;
- Conduct support groups focused on health maintenance and prevention on medical disorders;
- Provide assistance in transportation education and transport clients to medical appointments;
- Provide education on the recovery process, including conducting WRAP groups, and promote opportunities to enhance the ability of consumers served to move into recovery; and
- Arrange crisis intervention as needed.
You are a great fit for the Peer Health Coach role if you have:
- Must be a Peer which is a person who has personal lived experience and has used mental health services (past or present);
- High school diploma or GED; Certified Recovery Peer Specialist a plus.
- Ability to learn disease management, health maintenance, and WRAP curriculum;
- Ability to utilize supervision to clarify clinical concerns;
- A valid driving license with a clean driving record is required; and
- A high degree of empathy toward individuals with behavioral health disorders and the ability to connect with individuals of diverse ethnic and cultural backgrounds.
