Essential Functions
Perform the following duties for assigned caseload (in multiple service types):
- Maintain case records in accordance with licensing and Medicaid standards
- Crisis intervention and/or case management services
- Coordinate, develop, and implement Individualized Service Plans
- Conduct team meeting to incorporate all valid input into the Individual Service Plan development
- Provide trauma informed, person centered, psychosocial and psychoeducational skill building interventions with each client to achieve treatment goals
- Provide individual, family, and group counseling and therapy in keeping with approved credentialing and skill set
- Facilitate individual and family sessions to address identified goals
- Document all treatment activities to include phone calls
- Ongoing and consistent communication with affiliated case professionals to include school, criminal/juvenile justice, social services, MCO/Payor, and other mental health providers weekly
- Provide formal case updates and presentations to professionals in various settings (i.e., FAPT, IEP, Court)

Requirements
- QMHP-C, QMHP-Trainee (c/a), Licensed or Licensed Eligible (Through the Dept. of Health Professions)
- Must possess a Bachelor’s Degree (or Master’s Degree -preferred) from an accredited college or university in a mental health related field such as Psychology, Counseling, or Social Work.
- Minimum of 1 year of direct clinical experience working with children/adolescents and adults who have been diagnosed with a mental illness.
- Due to travel requirements must have valid Driver’s license with a good driving record.
Job Types: Full-time, Part-time
Pay: $20.00 – $30.00 per hour
Schedule:
- 8 hour shift
- Weekend availability
