MEDICAL SOCIAL WORKER HOSPICE
LOCATION: NORTH CITY/SUBURBS
HOURS: DAYS
This position is also eligible for a $5000.00 sign on bonus with 50% payable upon hire and 50% payable at 1year! You must be employed in the exact position at time of payout. Internal team members are not eligible for this sign on bonus at this time.JOB and UNIT SUMMARY: The purpose of this position is to help achieve the vision of Advocate Hospice Services by performing the duties of this position in a cost effective results oriented and customer focused manner. This position is a critical component of the interdisciplinary team and as such meeting individual performance expectations and contributions to team performance goals is required.Responsible for the development and enhancement of the psychosocial and counseling support services provided by hospice in collaboration with other health care providers and members of the hospice interdisciplinary team.Participates in the QA/CQI process for Advocate Hospice.Responsible for achievement of agency department and personal goals and objectives.Responsible for the assessment of psychosocial needs of patients and families as well as development of a social work plan of care reflecting problems interventions and outcome goals. Implements the plan of care in conjunctions with the interdisciplinary team.REQUIRED QUALIFICATIONS: Masters degree in social work from an accredited school of social workMinimum of one year experience in a medical clinical setting and/or hospice/palliative care.Social Work Licensure in state of Illinois (LSW)Experience working with extended families during times of excessive stress pending deaths and during the process of active dying of loved ones.
