Description of Position
Performs psychosocial assessments of patients/significant others to identify issues related to adjustment to illness, crisis intervention and discharge planning. Provides direct services of brief psychosocial counseling, discharge planning, education, and serves as community resources linkage to assist patients in achieving their optimal level of functioning, and to ensure the continuity of care. Engages in teaching, performance improvement, and other activities as assigned.
Qualifications and Requirements
Master’s Degree in Social Work from an accredited School of Social Work required. Current State of Hawaii Social Work License required. Two (2) years of Social Work experience at Master’s level preferred. Health care setting, geriatric and community resource experience desirable; or any combination of education and experience which would provide the necessary knowledge, skills, and abilities to meet the minimum qualifications to perform the essential functions of this position.
