Optimal Home Health is a well-established and renowned Home Health Agency that has been serving the community of Kern County for well over a decade. We have recently expanded our services to Tulare County and are looking for Medical Social Workers who are empathetic and willing to put the needs and well-being of our patients first and above all.
Job Title: Medical Social Worker – Part Time
Job Summary: To restore balance to a patient’s personal, family and social life to help maintain or recover in health and strengthen the patient’s ability to adapt into society.
Responsibilities & Duties:
- Assess/Evaluate the psycho-social status of patients related to the patient’s illness and environment and communicate findings to the RN
- Carries out social evaluations and plans intervention based on evaluation findings
- Maintains clinical records on all patients referred to social work
- Provides information and referral services for organization patients and families/ caregivers regarding practical and environmental needs
- Provides information to patients or families/caregivers and community agencies
- Provide counseling/help find community resources
- Maintains collaborative relationships with Organization personnel to support patient care
- Other duties as assigned
Required Qualifications:
- Graduate of a bachelor’s program in social work
- MSW certification required
- Possess and maintains current CPR certification
- Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements
Experience: Minimum one year in Home Health
Salary: As per market norms and experience
