Job Overview
Seeking a Licensed Social Worker to provide consultation to Administration regarding program planning, policy development and priority setting regarding social services in compliance with Massachusetts DPH and OBRA regulations and according to Facility needs. This is a split position between Beaumont at Northborough and Beaumont at Westborough.

Responsibilities
- Completing quarterly and annual assessments in accordance with MDS guidelines
- Conducting care plan meetings with families, residents and the interdisciplinary team
- Ensuring appropriate action is taken and services provided to resolve residents’ social and emotional issues
- Maintains all SW documentation related to Physical, Mental, Emotional, and Spiritual care of residents and families on daily basis
- Coordinates Community Health Link services (psychiatric services)
- Maintains contact with family, service providers, medical personnel as needed
- Reviewing clinical records for compliance with regulatory standards and quality of work provided
- Provides counselling and support to residents and families on variety of issues, including crisis management of agitation, aggression or other behavioral situations
- Completes and resolves grievances, and receives complaints
- Discussing clinical reviews of casework with social service staff and maintaining a log in accordance with Beaumont policies
Requirements
- Bachelor’s degree in Social Work
- Valid Social Work license- minimum of LSW (LMHC, LCSW, LICSW, LSW)
- Strong written and verbal communications skills
- Commitment to providing compassionate care
- Ability to work effectively as part of a team
