Job Summary
Summary:
Works in collaboration and partnership with chronically ill or high-risk? patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach to provide linkages for the various health and social needs of the patient. Provides assessment, treatment, group and individual therapy, and support services. Develop in conjunction with clinical team, an individualized treatment plan to address needs of the patient/significant others to support recovery. Work with individuals and groups to promote optimum mental and emotional health associated with chronic disease, depression, addictions and substance abuse, stress management; self-esteem; and aging.
