Job Overview
FUNCTION
The Intensive Case Manager is responsible for providing substance use screenings and assessments, mental health referrals, treatment planning, advocacy, escorts, and coordination of services for homeless clients who are suffering from co-occurring disorders (substance abuse and mental illness). This position will focus specifically on connecting clients in the Overnight Program to housing.
TOP RESPONSIBILITIES
· Conduct substance abuse screenings and assessments and develop treatment plans for AFC clients staying in the Overnight Program.
· Provide ongoing intensive case management services (crisis counseling, outreach, referrals, advocacy, escorts, and discharge planning) to clients with co-occurring disorders.
· Connect clients in Overnight Program to long-term housing.
· Perform evidence-based practices as assigned and written documentation of services provided.
· Facilitate regular occurring support groups at the Drop-In Center.
· Participate in weekly treatment planning meetings and provide knowledge and expertise about substance issues to direct care staff.
· Some weekend and evening hours required.
· Attend weekly staff meeting, supervision, and perform other duties as assigned.
EDUCATION REQUIREMENTS
[ X] Masters Degree
MSW degree required, or current and active enrollment toward said degree; NYS license preferred (LMSW/LCSW).
TECHNICAL SKILL REQUIREMENTS
Computer knowledge and competent writing skills.
PREFERRED QUALITIES
Must be knowledgeable in the skills of therapeutic engagement, substance use and abuse, and mental illness. Experience working with adolescent/young adult population required. Knowledge of psychosocial needs of LGBTQ/homeless population is helpful. Transgender & gender non-conforming people are encouraged to apply. Bilingual (Spanish) speaking encouraged to apply.
