About This Role 

The Intake Coordinator, internally known as an Access Care Manager, will answer incoming telephone calls to CPC, perform initial screening to determine level of care/ treatment need, document preadmission and associated paperwork to facilitate admission. The Access Care Manager will assist in addressing crisis situations and provide care management as needed. The Access Care Manager will assist in planning and coordinating intakes and transfers at various CPC outpatient locations.


Requirements
: BA degree in related fieldĀ Preferred: BA Degree in Mental Health or related field AND two (2) years of experience
