Inpatient Hospital Homeless Service Coordinator

Job Overview

As part of the Hospital to Housing multidisciplinary team, the Floating Inpatient Hospital Homeless Service Coordinator will work in both the hospital and community setting to engage homeless and/or housing insecure patients to identify and secure services for their physical, behavioral and basic needs outside of the hospital setting. The salary range is $36,500 to $40,000 per year.

RESPONSIBILITIES

  • Work directly with the hospital social workers and medical team to engage, accept and process referrals to the program from 3 partnering hospital systems, Barnes Jewish Hospital, SSM SLUH and Mercy STL
  • Conduct comprehensive, ongoing assessments of clients’ housing status, basic needs, healthcare needs and barriers that impact optimal well being in the community, preferably in person prior to discharge
  • Manage primary case load
  • Facilitate person centered, strengths base targeted case management services to individual clients within the office, hospital and in community settings
  • Refer and assist in warm hand offs to appropriate community agencies, consistent with evolving service planning
  • Coordinate and provide assistance to clients with making and keeping appointments and any other tasks medically necessary for client’s recovery process
  • Act as advocate and liaison, consulting with community agencies and providers to maintain coordination of care and transition
  • Collaborate with other members of St. Patrick Center (SPC) Hospital Program team during staff meetings and case consultations as scheduled
  • Maintain SPC client tracking system and client files (i.e. assessment, service plans, etc.) documenting client contact and progress to maintain a permanent record of client activity within the time frame required (24 hours)
  • Responsible for agency policy regarding work hours, building security and all written personnel standards
  • Perform within the adopted care models of SPC: Housing First, Trauma Informed Services and Recovery Oriented

KNOWLEDGE AND EXPERIENCE

  • Master’s degree in social work, counseling or a related field from an accredited university Required
  • Understanding of mental illness and substance use
  • Understanding of strengths base case management/wrap-around services
  • Knowledge and understanding of community agencies
  • Ability to work in high pace organization, travel between multiple locations
  • Housing First Model
  • Trauma Informed Care
  • Knowledge of federal, state and agency HIPAA policies
  • Must possess current Class E Driver’s license (Missouri residents only)
