EARLY INTERVENTION SERVICE COORDINATORS – Fee For Service
Diversified Children’s Rehab Services of WNY is seeking Early Intervention Service Coordinators to work in Erie County.
The Early Intervention Service Coordinator is responsible to provide assistance and services to enable an eligible child and the child’s family to receive the rights, procedural safeguards and services that are authorized under the Early Intervention Program.
Successful candidates will possess the following:
* One year of service coordination experience and an Associates degree in a health or human service field OR a bachelors degree in a health or human service field.
* Good organizational skills as well as time management skills
* An ability to communicate effectively with families and members of the child’s therapeutic team.
Must demonstrate knowledge and understanding in the following areas:
*Infants and children who may be eligible for early intervention services
*State and Federal laws and regulations pertaining to the early intervention
*Principles of family centered services
*The nature and scope of services available under the Early Intervention Program and the system of payments for services in the State
As travel is necessary, successful candidates will require a valid driver’s license.
Successful candidates will carry a minimum caseload of 30 children and be reimbursed per EI billable service.
