The Behavioral Health Crisis Manager is an integral part of the Behavioral Health Clinical team. In this role, you will be reporting to the Director of Behavioral Health and be responsible for working with patients, families, and other professionals such as urgent care staff and school administrators to address urgent behavioral health crises as they arise. The Crisis Manager will be responsible for responding to urgent clinical situations referred by self, parent, and others; identifying the severity of the issue; providing tentative diagnoses and brief interventions; assessing whether the patient needs emergency medical and psychiatric intervention; making referrals to the Behavioral Health program or community resources as appropriate; and providing appropriate follow up (patient, family, referring provider/source). Potential reasons for patient referral may include acute anxiety, depression, self-harm, suicidal or homicidal ideation, acute stress response, trauma, grief, bereavement, and behavior. You will be responsible for facilitating warm hand-offs to outside resources and agencies and may also be responsible for follow up to ensure patient and family have accessed necessary care and do not have additional unmet needs.

You will work in various settings, including telephone crisis counseling, our brick & mortar sites and for the school health program by providing clinical services to patients. The crisis manager also maintains a safe and efficient working and treatment environment per company policies, procedures, and best practices; communicates effectively with the treatment team to ensure safe, quality care is provided to all patients.

Description
  • Provides pediatric behavioral teletherapy as part of a large, multi-disciplinary behavioral health team in an innovative pediatric version of the collaborative care model.
  • Conduct face-to-face/Telehealth assessments with patients in their natural settings to determine their specific needs.
  • Perform initial evaluations of patient needs and develop customized treatment plans.
  • Educate staff and patients on stress and trauma coping mechanisms.
  • Provide individuals with the resources they need to reach personal goals.
  • Coordinate with social and medical agencies, healthcare providers and community outreach programs to provide patients with needed services.
  • Provide highest quality therapy services to pediatric patients with a focus on functional improvement, especially for depression, anxiety, ADHD, and grief/loss.
  • Participates in clinical team meetings to assess patient needs and coordinate services.
  • Completes required documentation and paperwork including clinical assessments, treatment planning, and updating of patient medical records within of 24 hours of patient visits.
  • Effectively use and manage patient registry to support clinical decision-making, including monitoring patient progress and facilitating team-based care, and updates registry with pertinent patient information in accordance with BH clinical team workflows.
  • Demonstrates cultural competency and commitment to patients.
  • Demonstrates commitment to the philosophy of the agency.
  • Assist in developing local promotional material to support referral partnerships.

Qualifications
Requirements:
  • Social Worker licensure required.
  • Knowledge of medical terminology.
  • Understanding the roles of the healthcare team.

  • Experience of community facilities and resources for discharge planning.
  • Superb written and verbal communication skills.

  • Experience in clinical informatics and delivery of medical care
  • Familiar with nuances of pediatric care, such as coordination with families/caregivers

Physical Requirements

This position may be remote and in-office at times. This position will require the incumbent to sit and stand at a desk, communicate in person and by telephone, frequently operate standard office equipment, such as telephone and computers, reach with hands and arms, and bend or lift to 25 pounds.
