Job Overview

Segue Inc. is a non-profit outpatient mental health facility that has been providing quality mental health services to the citizens of Jackson & Hillsdale counties in Michigan for over 30 years; offering intensive and essential services and programs.

 

The Community Mental Health Advocate provides client advocacy, monitoring, referral and linkage, obtainment of entitlements, social support, and emergency/crisis management services to ACT. This position is Monday through Friday, from 8:00 AM – 4:30 PM; rotated on-call is required.

REQUIREMENTS: The successful candidate for this position must be fully vaccinated against COVID-19 as a condition of employment.

QUALIFICATIONS:

  • Qualified Mental Health Professional with specialized training (including fieldwork and/or internships with the academic curriculum where the student works directly with persons receiving mental health services) or one year experience in treating or working with a person who has mental illness; AND
  • Possesses a Bachelor?s degree in Education, Social Work, Criminal Justice, Human Services or is a Registered Nurse.
  • Able to type 35 wpm with a high degree of accuracy
  • Experience working with electronic medical record software preferred
  • Lives within a 45-minute radius of the local hospital to meet on-call responsibilities, if required

 

 

ESSENTIAL JOB RESPONSIBILITIES:

A. Provides clients with face-to-face contacts as required by the treatment plan.

B. Advocates for, and assists consumers to acquire needed community resources and entitlements.

C. Assists consumers in learning community skills.

D. Monitors client needs, safety, and well-being.

E. Provides skills training.

F. Participates in daily report and team planning meetings and makes recommendations regarding treatment.

G. Reports suicide gestures or ideations and any related issues to the Clinical Team Leader immediately.

H. Uses personal vehicle to visit and transport consumers.

I. Provides 24-hour emergency/crisis on-call coverage on weekdays, weekends, and holidays on a rotating basis, as assigned.

J. Helps consumers to understand their mental health diagnosis. Uses mental health certifications when working with consumers.

K. Provides education on mental illness management and recovery skills to consumers and supports.

L. Required to work additional hours or other duties as directed by Administration.

 

 

COMPENSATION AND BENEFITS

The rate of pay is $17.00
