Community House is looking to fill a position for- Mental Health Housing Support Specialist Coordinator.
Position: Clinical Coordinator for Mental Health supported Housing Program

Employment Type: Full-Time (35 hours/week) Monday – Friday 9:00 AM – 4:00 PM
Full medical, dental, disability, 25 plus days off per year to start.
Mental Health Housing Support Specialist Coordinator Job Summary:
This position works directly under the Clinical Director to coordinate programming and services for the Standard Supported Housing program, activities which may include: coordinating staff schedules; supervising Housing Support Specialists; interviewing potential clients and coordinating client placements into the program; assuring completion of daily and monthly paperwork by the team; development of groups and activity calendars for the program; individual work with clients within the program; coordinating with larger treatment team of case managers, psychiatric providers, nurse, etc; coordinating with hospitals and other providers on client referrals; and is responsible for meeting program timelines and guidelines.
This position works in a team approach to provide wrap around support services to adults with severe and persistent mental illness, assisting them to successfully live independently and integrate into the greater community.
Works in people’s homes to assist them in developing the skills needed to be successful in living independently and in the community. Provides one-on-one and group services as well as crisis intervention, assistance with appointments, outings, etc…
This position works collaboratively with the Mental Health Housing Support team and a larger clinical team of case managers, psychiatric providers, day treatment staff, etc. to assure that each client is actively working towards treatment goals, while coordinating multiple levels of Mental Health Housing Support activities and duties.
Must be a self-starter, able to work autonomously when needed, a good team player, and passionate about working with this population.
**After Hours Crisis work may also be required of this position**
Minimum Qualifications:
- Must have at least 2 year of professional clinical experience working with adults who have severe and persistent mental illness in a community setting/program development experience a plus.
- Master Degree required
