Position Summary
Project LINK provides specialized assistance to substance-using and at-risk pregnant, postpartum and parenting women and their children. The Case Manager for Project Link will provide intensive case management and home visits to pregnant and parenting women who are “at-risk” or are currently abusing substances. Case management services for the individual can include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, coordinating, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions. Case Managers assist individuals with their basic physical, financial, social and personal needs.
Required Education and Experience
- Bachelor’s degree from an accredited college/university in an applicable human services field. Or relevant education, which indicates that the individual possesses the required knowledge, skills, and abilities.
- A minimum of one year of relevant mental health, developmental disability or substance abuse work experience or a combination thereof that demonstrates candidate possesses the required knowledge, skills and abilities; and/or an equivalent combination of education, training and experience that provides the individual possesses the required knowledge, skills, and abilities.
Related Knowledge, Skills and Abilities
Knowledge of case management practices and resources available to help individuals with mental illnesses and/or substance abuse disorders and their families/care givers in the community; Knowledge of military life and veterans services preferred; Services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, developmental disabilities, and substance abuse treatment programs; The nature of serious mental illness, developmental disabilities, and substance abuse depending on the population served, including clinical and developmental issues; Different types of assessments, including functional assessments, and their uses in service planning; Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination; The service planning process and major components of a service plan; The use of medications in the care or treatment of the population served; All applicable federal and state laws, state regulations, and local ordinances; To drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client @other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; To participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed; Willing and available to work modified schedules (weekends, holiday, etc.) as required; Demonstrate positive regard for individuals and their families (e.g., allowing risk taking, avoiding stereotypes of developmentally disabled people, respecting individuals’ and families’ privacy, believing individuals can grow).
