The City of Norfolk is seeking a Case Manager III – Service Coordinator to join the Early Intervention Team within the Norfolk Community Services Board.
Provides Service Coordination/case management services to children, birth to three years of age, with developmental delays, diagnosed conditions or atypical development & their families by assessing, planning, linking, monitoring, and advocating for them in response to their changing needs. Completes clinical documentation in accordance with agency and regulatory guidelines.
The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the CSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Work Location: Robin Hood Road, Norfolk, Virginia
Work Schedule: Monday – Friday 8:30am-5:00pm
- Provides early intervention service coordination from initial referral through transition/discharge.
- Develops an Individualized Family Service Plan (IFSP) based on assessment information conducted by a multidisciplinary team and in collaboration with the client’s family.
- Reviews and updates the IFSP at least every 6 months and conducts a complete revision of the IFSP annually.
- Maintains documentation in accordance with State regulations.
- Complies with Part C, Medicaid, DBHDS, and City requirements.
- Contacts families and/or service providers monthly by phone or in person.
- Links children and families to appropriate resources and supports.
- Monitors progress, service delivery, and family satisfaction with services. Provides practical counseling and education to guide the family/caregiver in problem solving strategies and develops a supportive relationship with the family.
- Performs crisis intervention to resolve critical situations.
- Performs related work as assigned.
Two years experience in a human services field related to the areas of assignment (Early Intervention) required.
Signing Bonus: This position is eligible for a one-time $5,000 signing bonus for applications received on or after February 22, 2022 until June 30, 2022. The signing bonus will be paid in two (2) increments: $2,500 upon completion of 60 days of employment, and $2,500 upon completion of your probationary period. To receive the signing bonus, you must be an active employee in good standing.
- Valid Driver’s License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
