The Case Manager III provides social services to psychiatric patients. May provide services to geriatric patients and persons with developmental disabilities. Works under the supervision of the Director of Social Services. Acts as treatment team liaison among all entities including Big Spring State Hospital, patient families and community agencies for the purpose of assessment, education and coordination of patient discharges. Is accountable for completing documentation within designated timeframes and according to applicable standards and investigating and coordinating discharge plans. Is committed to the Big Spring State Hospital philosophy of psychiatric rehabilitation and family services. Must have physical, mental and emotional health necessary to carry out essential functions of this position
Attends work on a regular basis accordance with agency leave policy and performs other duties as assigned.
Participates as a member of an inter-disciplinary team.
Completes Social Assessments
Assessments comply with Big Spring State Hospital approved outline and cover all necessary criteria, as possible.
Social assessments include information from family or other collateral sources as available.
Testifies at probable cause court hearings.
Provides patient/family education concerning community resources, where and how to obtain assistance when needed and other issues addressed in treatment plan.
Facilitates continuity of care.
Obtains information from community providers concerning prior community care and recommendations for treatment.
Information is obtained at admission or as soon as possible thereafter.
Discharge plans are coordinated with community providers and include recommendations for appropriate referrals or other continued treatment.
Clinical Information is recorded in the electronical medical record according to Departmental timeframes, Joint Commission and Medicare Standards.
Participates in required training within 30 days of due date including, but not limited to, CPR, Seizure, TIPS, Infection Control, Emergency Response, etc. based on TDMHMR Minimum Training Requirements for assigned job group.
Participates in facility in-service, departmental meetings, and continuing education offerings as requested.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Graduation from a four-year college or university.
Major coursework in behavioral sciences, social sciences, humanities, or a related field is generally preferred.
