Master’s
- Responsible for managing the care and coordination needs of clients that have co-occurring primary and behavioral health conditions and who have been enrolled in the Behavioral Health Home.
- Must maintain an acute awareness of services, therapeutic activities and supports available within and outside of the agency that may be of benefit to individual enrollees and their families.
- Help assure access to services, benefits and entitlements, monitor and coordinate the delivery of services with other treatment team members, service providers and community organizations.
- Provides care management and care coordination for enrollees with mild, moderate and severe illness, under minimal supervision. \
- Leads the enrollee’s care and collaborates with members of the health care team to empower enrollees to manage their chronic behavioral and medical conditions.
- Assists those, who are at risk for developing chronic conditions, to minimize these risks.
- Serves in an expanded health care role to collaborate with PCP, psychiatrist, nurse, peer and other health care team members with enrollees to ensure the delivery of quality, efficient, patient centered, preventative and cost-effective healthcare services.
- Assess, plans, implements, monitors, and evaluates delivery of individualized enrollee care with the goal of optimizing the patient’s health status.
- Provides self-management support and patient education. Individual programs may have additional requirements.
- Services are delivered in the office setting and the community. Regular working hours are Monday through Friday 8:30 a.m. to 5:00 p.m. Some evening or weekend hours will be required.
- Must possess a Behavioral Health degree with a corresponding Michigan license (LMSW, LLMSW, LPC, LLP, LP).
- Candidates must meet the criteria for Qualified Mental Health Professional, and Child Mental Health Professional within 120 days of hire
- Must have at least (1) one-year post-Master’s experience in treating, working with, or providing behavioral health services to adults and children with severe mental illness.
- Experience in a medical setting is preferred.
- Must have a good driving record and obtain a Chauffeur’s License.
- Must have the ability to engage mental health consumers, their families and supporters.
- Must be an expert in helping consumers identify their service needs and preferences and helping them build on their individual strengths, resources and supports to navigate the network of services available at the agency and in the community.
- Must have an orientation toward providing community partnerships for those served.
- Additional requirements include a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of those with mental illness, their families and providers.
- Candidates must be able to work independently, demonstrate effective organizational, verbal and written communication skills.
- Must have the ability to appropriately handle crisis situations.
- Must have excellent organizational and attention to detail skills in order to track and prepare data accurately and timely.
- Must have the ability interpret a variety of instructions furnished in written, oral, or schedule form.
