Summary:
This position will provide support to medical, behavioral and social support staff to determine needs, eligibility and services for members. Conduct home visits within assigned territory to complete screening and risk assessment interventions per program guidelines.
Job Responsibilities:
- Identify special needs members through the completion of health screens and other resources.
- Complete comprehensive assessments, medication reconciliations and plans of care. Update plans of care. Send plans of care-to-care team participants.
- Connect members to PCPs and specialists and ensure members are following up with their physicians.
- Address social needs such as housing assistance, homelessness, food insecurity, etc.
- Provide education on disease or condition management
- Work with community outreach/member advocates to coordinate member care.
- Educate providers and community resources on program components and available support services.
- Educate members with special needs to foster compliance with program and positively impact outcomes.
- Conduct site visits as appropriate for programs and provide support to other special programs.
- Assist with modification and management of care plans under the direction of a Registered Nurse in conjunction with member, medical consenter or caregiver, Department of Family and Protective Services (DFPS) caseworker and family, if applicable, as well as the managing physician.
Required Education/Experience:
- Bachelor’s degree in Social Work, Health Services, Behavioral Science, or related field, or equivalent experience.
- Experience in managed care and/or behavioral healthcare setting.
- Microsoft office, Excel
Preferred skills:
- 2+ years of experience in managed care and/or behavioral healthcare setting.
- Experience with pediatric conditions, patients with special needs, social services for children, DFPS or child welfare system preferred.
Shift: 8:00am-4:30pm
