About Us

Petaluma Health Center is a nationally recognized, award-winning community-based 501(c)(3) non-profit corporation and Federally Qualified Health Center (FQHC) committed to ensuring that all people, and especially medically underserved populations, have access to high-quality, patient-centered health care. PHC provides comprehensive medical, dental and mental health services to approximately 36,800 patients (with over 200,000 visits) per year across our 10 sites in Sonoma and Marin counties.

PHC is widely recognized for our innovation and has been on the leading edge of team-based care since 2007, emphasizing overall wellness and the value of the patient/provider relationship. Recent recognitions include:

PHC’s mission is to ensure access, and to provide high quality prevention-focused health care for the communities we serve. We aim to foster a healthy community where all individuals reach their highest potential for health and well-being. Petaluma Health Center has a HPSA Score of 20 – excellent for several loan repayment programs! We provide loan repayment assistance for all candidates.
