Job Overview
Principal Responsibilities:
1. Assess Guardianship and Financial Management clients in their home, hospital or other institutional setting as appropriate
2. Screen and assist clients with applications and re-certifications for entitlements and benefits;
advocate on behalf of clients and arrange for provision of services; develop, implement and follow up on care plans
3. Provide supportive counseling to clients as needed
4. Maintain ongoing communication with client, family members and Guardianship team as appropriate
5. Make home visits to elderly Holocaust survivor clients for the purpose of assisting them with financial management tasks, including sorting of documents and mail, writing checks, monitoring financial transactions and reconciling bank records monthly
6. Assist Administrative Assistant and/or Program Director with tasks related to Guardianship clients’ relocation; assist in obtaining and filing Court and Guardianship documents.
7. May be asked to provide occasional on-call coverage for Guardianship emergency line
8. Maintain complete and accurate case notes utilizing the program’s data system; prepare required documentation in case file within organizational time frames
9. Maintain and report on statistics according to Selfhelp standards, utilizing the program’s data system and other forms of record keeping as needed.
10. Actively participate in supervision, staff meetings and Guardianship team meetings; share any emergent issues with supervisor in a timely manner
Working Conditions/Physical Demand: Business office environment with phone and computer use, travel via public transportation. This position may have the ability to work from home 2-3 times per week.
- Master’s Degree (MSW); LMSW preferred
- Experience with geriatric population preferred
