Founded in 2008 by pre-eminent psychiatrists and psychologists in the eating disorder space, ERC Pathlight now treats over 6,000 patients per year, operates more than 30 facilities in 7 states and delivers tele-healthcare to patients nationally. We offer the most comprehensive treatment program in the country for patients who struggle with eating disorders, mood and anxiety and trauma-related disorders.
How you’ll serve our patients
As a clinical professional at ERC Pathlight, you can expect to be a part of team that delivers hands-on treatment, therapy sessions and/or support to our patients in one of our many centers around the nation, or virtually through our tele-behavioral health platform.
We are looking for a case manager to collaborate with treatment teams and external providers to ensure patients receive the best treatment and experience as they transition through our system of care. Our system of care includes residential, partial hospitalization and intensive outpatient services.
You will serve patients daily in a multitude of ways:
- Finalize completed treatment plan updates to be sent to referring providers weekly
- Track patient movement throughout the system of care
- Support with aftercare coordination for disposition planning and communication with outpatient treatment teams to ensure continuity of care
- Obtain patient consent to participate in outcome surveys and administer surveys to adults, adolescents, and parents
- Support completion of FMLA paperwork for patients and family members
- Duties could also include:
- Attend team meetings in which patient cases are discussed
- Orienting new patients and families to the program
Must haves:
- Bachelor’s degree in psychology, social work or related field required
- Experience in behavioral health preferred
