Job Description:

We are looking for an energetic, dependable, and positive team player, preferably with therapeutic, educational and teamwork experience to partner with the primary care team to provide collaborative care and therapy via our Integrated Behavioral Health program to our patients and families.

As an integral member of the patient care team, the behavioral health specialist (BHS) promotes child development, behavioral health, and family resilience by providing the clinic with a systematic approach to both therapy and behavioral health education.

The BHS plays a key role in ongoing Behavioral Health Integration and Patient-Centered Medical Home initiatives, As an IBH team member, the BHS will help develop systematic identification, care plans, and follow-up for behavioral health patients, as well as identify major partners providing mental and social health services in the community. Additional training and support is provided through our partnership with Seattle Children’s Care Network faculty and clinicians.

The BHS provides comprehensive screening, assessment and behavioral health care to clinic families through the use of a team approach. They are embedded into primary care teams, sharing patient panels and supporting robust screening and treatment for the care team’s entire patient population.

Primary Responsibilities:

Direct Patient Care:

  • Conduct and interpret screenings of children for emotional, behavioral and developmental needs.
  • Develop comprehensive, family-driven care plans based on family strengths, priorities, and needs.
  • Provide trauma-informed, family-focused psychotherapy, and parent education
  • Help stabilize families with a mental/behavioral health crisis.
  • Proactively identify patients who require behavioral or developmental intervention
  • Participate in treatment after warm hand-offs from PCPs who have identified BH needs.
  • Evaluate mental health crises and apply appropriate interventions.

Collaboration with Care Team Members:

  • Complete health assessments and care plans that address developmental, behavioral, and social service needs.
  • Consult with the primary care team on the patient’s care plan, needs, and goals.
  • Participate in case review with the clinic psychiatric consultant.
  • Participate in scheduled primary care team meetings and pre-shift huddles.
  • Assist the primary care team in developing care management processes such as the use of guidelines, management techniques, case management, and patient education.

Care Coordination:

  • Help to triage and refer patients who require psychiatric assessments, long-term services, developmental support services, or other specialty mental and social health services.
  • Participate in care conferences with PCP and Seattle Children’s Hospital psychiatric consultant.
  • Collaborate with schools and early childhood centers of care on behavioral and educational assessments and plans of care.
  • Serve as liaison for the Department of Children and Families to ensure children and families with DCF involvement have all needed support in place.

Tracking:

  • Track all clinic patients with behavioral and developmental needs.
  • Create a registry and tracking system that meets grant reporting requirements.
  • Document all encounters, patient progress, and treatment recommendations in the patient’s electronic health record.

Education

  • Participate in Integrated Behavioral Health Collaborative to receive additional training, communicate with partner clinics and behavioral health specialists

Other Duties as Assigned

Job Requirements:

  • Master’s Degree and license in Social Work (LICSW) or other closely related field
  • Clinical training in child, adolescent therapy, including CBT.
  • Strong ability to connect, support, and partner with culturally diverse families
  • Organized, resourceful self-starter with an ability to set priorities and meet deadlines
  • Strong ability to collaborate and work in a team
  • Excellent written, oral and interpersonal communication skills

Preferred:

  • 2+ years therapy experience- pediatrics or young adults preferred.
  • Training in trauma-focused therapy, parent-child psychotherapy, and integrated behavioral health care
  • Experience in working with schools, trauma, care management, and crisis teams, or working on a treatment team within complex systems
  • Experience with medically complex children with severe behavioral problems and/or developmental disabilities.
  • Comfortable with the pace of primary care, that is, shorter sessions, short interventions
  • Cultural and linguistic diversity highly preferred
  • Experience with collaborative care concept, terminology and billing
  • Experience with evidence based practice and working with a multidisciplinary team
  • Experience with the King County MHP and Washington State Mental Health WACs.
  • Population health perspective with interest in Patient Centered Medical Home transformation

Job Type: Full-time

Pay: $70,000.00 – $85,000.00 per year
